Role description and task arrangements
As a youth counselor and case director, you visit residents: parent(s) and children/youth. You engage with them about their care needs. You get to know them from a broad perspective (all areas of life) and try to find a solution together with them that does justice to their own responsibility. This not infrequently leads to a support plan in which the resident is their own director as much as possible, but in which you support them or have to direct them yourself.
You do not provide help yourself, but together with the resident you try to activate others, preferably from the resident's own network, to provide that help.
In extreme cases, when the resident's own strength and other available services are not sufficient, you arrange professional support for which an indication is required. In these cases you issue a decision (which must be purposeful and lawful) and evaluate it with the resident and provider.
Within the cluster you work closely together with the other youth counselors, Wmo counselors, work and income counselors and education counselors (compulsory education and student transportation). There is also cooperation with colleagues from a number of chain partners in the supporting field such as: welfare, general social work, volunteer organizations, etc.
This assignment arose after peak workload due to sick employees and increasing waiting times. We are looking for people who can be deployed quickly and have a lot of experience with multi-problem cases and can act quickly in crisis situations.
