Role description and task arrangements

Wijkteamsmedewerker WMO
36 hours per week, Enschede

For Wijkteams Enschede we are looking for an enthusiastic and professional colleague who would like to work in a dynamic work environment. You will be part of a motivated team with driven colleagues where there is room for new initiatives and ideas.

Who are we?
You will work at Wijkteams Enschede and together with other WMO and Wijkteam employees you will form a team. The teams are divided by district. At this moment there is vacancy in stadsdeel Centrum. Each district has its own dynamics. You will work closely with colleagues from the front and back office, Policy, Legal Affairs, Enforcement and with partners in the neighborhood. Wijkteams Enschede works based on our vision of an inclusive society. We stand for a society in which everyone can participate as fully as possible, regardless of gender, age, cultural background, talents and limitations. We believe it is important that you promote and endorse this vision professionally. The core values for our actions are connection, trust, eager to learn and personal.

As WMO district team member...

- inform, advise and support the resident in making choices in the field of housing, welfare and care. To this end, you engage in conversation (e.g. by making a home visit), make the request for help concrete and check to what extent the resident is co-reliant;
- you deal with applications from residents, assess these and, if necessary, make an indication. This may relate to home adjustment, wheelchairs, transport, domestic help, day care or guidance;
- you maintain internal contacts and also have contact with care providers and social workers in order to be able to deploy the requested care as efficiently as possible;
- you have a signaling role with regard to (neighborhood) developments and innovations and you keep the social map up to date.
What do we ask of you?
In the first place, you have a warm heart for Enschede and our residents. After all, that's what you do it for! With your open mind you pick up things quickly, connect with people inside and outside the organization and seize opportunities to bring about real change. In addition, you know how to motivate the client and you offer support in exploring possibilities to use your own strengths and network. In doing so, you work professionally and quality-oriented. Furthermore, we ask

- demonstrable HBO working and thinking level;
- knowledge and experience in one or more of the following areas: occupational therapy, physiotherapy, social work, nursing, psychology;
- knowledge of and experience with relevant laws and regulations;
- knowledge of the social map;
- good analytical skills;
- excellent social and communication skills;
- experience with automation
