Role description and task arrangements

We are looking for you!
Schiedam needs you! The District Support Teams in Schiedam work from 6 locations in the city where you, as a professional, will support adults (18+) in solving social (multi)problems. To complement our expertise and commitment to the increasing requests for help from residents with staple and multi-problem issues, we are looking for solid professionals who take action and find sustainable solutions in cross-domain cooperation. Thereby, navigating between motivating, nudging, intensive and proactive approach is essential.

Although professionals are positioned in the neighborhood teams, you work on a more urban and specialized level in terms of bemoeizorg and intensive care. Important partners are VTRR, Central Reception, Stroomopwaarts (participation, income and debt assistance) and the municipality of Schiedam with regard to mediation care, PGA, hoarding and care & safety directors.

Job Profile
The purpose of this position is proactive counseling approach for Schiedammers with multiple stack issues. Specifically, housing-related assistance requests prompt intensive support and require sustained counseling for dealing with underlying behavioral patterns, care avoidance and/or misunderstood behaviors.  

The following skills are important :
  • Connection and contact with target group: LVB, multi-problem, stressful (family) situations and/or misunderstood behavior
  • Case management; you implement and set the actions, jointly set goals and divide the tasks, either with client, with you as pathway supervisor and/or with collaborative partner.
  • Analyze problems, address preliminary issues so that interventions are targeted and effective.
  • Stabilize and direct to appropriate (follow-up) care, incl. motivate client.
  • Intensive collaboration with parties, and that means follow-through, customization and creative mindset to deal with bureaucracy
  • Hands-on mentality, outreach and incisive: able to change working methods.
  • Can (over)switch between operational and tactical levels.
  • You are familiar with behavioral intervention of Mobility Mentoring, "foster care" and hoarding issues.

It is important that you are able to see different aspects of the risks, problems and solutions within supervision and dynamically anticipate and manage them.

 The candidate has knowledge of:
  • Theoretical knowledge regarding developmental psychology
  • Social security and basic services in the region
  • Experience with psychosocial problems and counseling thereof
  • Social developments and issues of this speciic target group

Furthermore, you should be familiar with adjacent sectors; preventive youth health care, parenting and growing up support, mental health care and issues of persons with (mild) intellectual disabilities.

What do we expect from you?
  • You have a college degree (social work, MWD, SJD, SPH or pedagogy)
  • SKJ registration preferred
  • Minimum of 4 years work experience in a similar position
  • Analytical ability to translate (complex) requests for help into an assistance plan and to arrive at sharp analysis and interventions (on safety) in crisis situations
  • Knowledge and understanding of the social map, recent assistance methodologies and assistance offerings
