Role description and task arrangements

Assignment Description

In the Southeast Utrecht (ZOU) region, five municipalities are working closely together in the areas of Youth, Social Support Act (Wmo), and Participation.
We have also been working together in the region since 2023 on the Integral Care Agreement (IZA) and the Healthy and Active Living Agreement (GALA), implementing various projects and chain approaches. In September 2025, the Supplementary Care and Welfare Agreement (AZWA) was concluded, which broadens the scope of the IZA and will run until 2028.

The AZWA focuses on the following areas of life: mental health, healthy aging, growing up with opportunities for a healthy lifestyle, and reducing/preventing health inequalities. For each area of life, there are so-called basic functionalities (chain approaches), such as Promising StartSocial Referral, Tackling Overweight & Obesity in AdultsFall Prevention for the Elderly andAccessible Support Centers.
To make this a success, municipalities need to provide a range of accessible facilities: a basic infrastructure. Municipalities work together with the Municipal Health Service (GGD), Youth Health Care (JGZ), social services, primary care providers, mental health services (GGZ), and informal care providers.

The above-mentioned development calls for a strengthening of regional control over the basic functionalities of AZWA/GALA chain approaches, in conjunction with other municipal tasks in the field of prevention and health promotion. In particular, this also involves coordination with the regional procurement policy, with the aim of making the transition to normal life within the social domain.

We are looking for a regional prevention coordinator for this purpose.
 

Period and commitment

We are looking for a regional coordinator for 20–24 hours per week, for the period from January 26, 2026, to January 26, 2027. If circumstances require, the client may terminate the agreement earlier, with one month's notice.
 

Work

We are looking for a regional coordinator for GALA chain approaches/basic functionalities and prevention who:
  1. Providing tactical and strategic leadership, establishing connections between parties and domains, and supporting municipalities;
  2. Advises on the long-term implementation of regional AZWA coordination, in response to developments in local and regional partnerships, political decisions on AZWA and funding, and the role (and implementation) of the Municipal Health Service (GGD).
  3. If necessary, advises on the establishment of new consultation structures in the ZOU region.

More specifically, this concerns the (further development of):
  • Process management of the integrated and cross-domain implementation of the chain approaches/basic functionalities and other agreements from the GALA and AZWA, with attention to the position of municipalities;
  • Coordination with and between partners in healthcare (particularly primary care) but also informal care, welfare, education, sports, housing, etc. (bringing worlds together);
  • Ensuring that regional priorities are aligned with local implementation in municipalities;
  • Advising and supporting municipalities and chain partners in monitoring and evaluation;
  • Collection of regional-level data on the progress, results, and effects of prevention approaches and periodic reporting on this to municipalities and chain partners;
  • Advising and supporting municipalities and chain partners in the selection and implementation of proven interventions;
  • Support in translating national agreements and programs into local and regional policy.
 

Frameworks and preconditions

Training Requirements

  • HBO/WO working and thinking level
 

Experience requirements

  • At least 2 years of demonstrable work experience in a similar coordinating role at a tactical-strategic level within a regional partnership.
  • At least 3 years of demonstrable work experience as a (coordinating) project manager IZA/GALA.
 

Additional requirements

  • Being ableto combine substantive expertise with organizational and interpersonal skills. The position focuses onregional coordination, networking, and quality assurance, while local implementation remains the responsibility of the municipalities
  • Familiarity with the U16 and/or Southeast Utrecht region (demonstrated by work experience or a brief explanation).
 

Financial terms

The maximum hourly rate is €110, excluding VAT, including commuting expenses.
