Purpose of the function

• Planning, coordinating, and providing specialized nursing care to hemodialysis and peritoneal dialysis patients.
• Independently performing renal replacement therapies (hemodialysis, peritoneal dialysis, and CVVH) in accordance with the medical treatment plan.
• Monitoring vital signs and intervening promptly in the event of complications.
• Supporting and training patients to achieve as much independence as possible.
• Contributing to scientific research, education, and the advancement of expertise.

Result Area 1 – Nursing Care Process

• Assessing care needs and developing a nursing care plan.
• Providing nursing care and dialysis treatments in accordance with protocols and the treatment plan.
• Monitoring the patient, interpreting treatment data, and adjusting the treatment as needed.
• Providing information, instruction, and psychosocial support to patients and their loved ones.
• Evaluate the care and adjust the nursing care plan based on the patient's progress.

Result Area 2 – Coordination of Care

• Coordinating the multidisciplinary care process.
• Coordinating nursing care with physicians and other disciplines.
• Recording and monitoring the progress and quality of care.
• Participate in multidisciplinary meetings.

Result Area 3 – Operational Coordination

• Act as the team leader's substitute when he or she is absent.
• Planning admissions, staffing, and patient logistics.
• Chairing multidisciplinary meetings and patient case conferences.

Result Area 4 – Quality Assurance

• Identifying areas for improvement in healthcare.
• Contribute to quality improvement, protocol development, and the implementation of new procedures.
• Supporting and conducting scientific research.
• Actively participate in department meetings.

• Mentoring students, interns, and new employees.
• Transfer of knowledge and skills.
• Contributing to the training of dialysis nurses.
