Role description and task arrangements

ICU Nurse
 
Job Objective:
- To coordinate and implement the total nursing care process of the intensive care patient to achieve a nursing intensive care process (CBO level 3) within the framework of the patient care policy and the medical treatment plan. Whereby promoting that the patient with an imminent or present severe impairment of one or more vital signs receives the care and monitoring that his or her situation requires as soon as possible.

HIGH COMPLEX Nurse Practitioner INTENSIVE CARE CARE PROCESS (equivalent to senior ICU nurse)
Nursing intensive care care process planning:
 Inventory and record all relevant patient data (diagnosis, admission indication, personal data) to arrive at a planning of high complex/acute nursing care that includes problem, goal, activities and evaluation times based on the patient's care needs, medical treatment plan, protocols, procedures, standards and professional expertise.
 
Activities:
  • aggregates relevant, available information
  • Conducts interviews with the patient (and/or family/neighbors) in order to gather relevant information
  • Observes the patient in order to gather relevant information
  • Conducts interviews with the patient (and/or family/neighbors) in order to discuss the proposed nursing plan
  • Prepares a nursing plan (consisting of problems, goals and actions) appropriate to the patient's care needs
  • Participates in multidisciplinary consultation
  • Ensures connection of nursing care on pre- and post ICU phase and integration with other disciplines, likewise when transferring to another department
Implementation of nursing care process
Perform high complex/acute nursing care based on the established nursing plan such that the patient receives the nursing care that leads to realization of treatment and nursing goals.
Activities:
  • Provide needs-based education to the patient, and/or family/neighbors
  • Provide instruction and/or education to patient and/or family/neighbors
  • Perform nursing operations and interventions, even when standard procedures are not available
  • Assist with, and if necessary take over, physical care
  • Monitor, support and, if necessary, independently take over vital functions
  • Independently intervene in acute situations
  • collection and interpretation of data (including vital signs) based on laboratory values and physical examination, particularly with regard to respiration and circulation, and through clinical observations and using specialized medical equipment
  • Administration of special medication, where there is an increased risk of serious side effects/ complications
  • Provide psychosocial care to patient and family during the acute or chronic disease process, the dying phase or donor procedure.
  • Providing consultations related to the primary process
Evaluation implementation nursing care process
Continuously coordinate the quality and effectiveness of the highly complex/acute intensive care provided with the individual patient and other caregivers based on the achievement of individual treatment and referral goals to achieve systematic recording of data that allows for adjustment of the existing care process.
Activities:
  • Ensure systematic reporting
  • Have conversations with the patient (and family/neighbors if necessary) about progress and any bottlenecks in care delivery
  • Observe the patient in order to gather relevant information
  • Adjusting the nursing plan as necessary (after coordination with other disciplines, the patient and, if necessary, family/neighbors and with input from colleagues)
COORDINATION OF THE CARE PROCESS (equal to senior ICU responder)
Coordinating and organizing the execution of the total intensive care process of own patients. In cooperation with all disciplines involved in order, based on the treatment and nursing goals, to provide the patient with a tailored and integrated range of care and treatment within the time set for this purpose.
Activities:
  • Per shift, plan and execute own work, arising from the nursing plan, in coordination with other (senior) ICU nurses and other disciplines.
  • Inform the person ultimately responsible for coordinating overall patient care in the department of the progress of care tasks
  • discuss adhoc issues regarding medical or paramedical policy during the shift with relevant professional
  • Participate in discussions regarding nursing, individual patient care
  • Coordinating the implementation of care activities with other health care providers
  • Initiate and participate in multidisciplinary discussions on individual patient care
  • working as the person with final responsibility in the department: a.o. planning acute admissions with the physician on duty and solving (short term) service list problems.
  • If necessary, participation in the resuscitation team
QUALITY CARE (different from senior IC nurse).
Depending on the context and needs of the department, the nurse will contribute the development of non-patient related areas such as quality assurance, protocol development, professional development, research, etc.
This involves identifying the bottlenecks and discussing with manager the desired situation and contributing to the realization of the desired situation.
Activities:
  • Identify and report bottlenecks in care delivery
  • Active participation in (team) discussions or the departmental quality group, this includes bringing in proposals for improvement.
  • Contribute to and keep abreast of (the development of-) new protocols, practices and methods of treatment and care.
  • Co-execute improvements in practice
  • Contribute to the practical conduct of scientific research.
  • identifying and reporting errors and near misses
Powers
  • The ICU nurse works within a complex situation, where a strict separation in medical and nursing responsibilities and authority does not prove feasible in all situations.
  • Within the field of tension of the above, the IC nurse is expected to act in accordance with prescribed medical and nursing guidelines/protocols, while also being responsible for minimizing risks to the patient. In the event of acute vital signs, she regularly acts independently and crosses the medical domain.
Relations
  • The ICU nurse interacts with patients' loved ones/relatives, fellow nurses and other caregivers regarding the execution of the primary process and collaborates with officials from the medical and paramedical disciplines whether in training or not.
  • The ICU nurse consults with peers, educators, researchers and policy staff for the purpose of optimizing patient care, research and education.
